About Us

Revive Dental Sleep Solutions is owned and operated by Heidi Johnson, DDS, ABDSM (if possible can this be linked to my bio on the other page too?). At Revive we help facilitate treatment prescribed by your physician and or PCP.  We fabricate custom oral appliances used to manage obstructive sleep apnea.

We also provide identification and relief for TMJ dysfunction, snoring, bruxism and more.

What To Expect

30 Min Consult Appointment

Consult appointment to verify treatment needs, that you qualify and are a candidate for a sleep appliance. We will review your health records and acquire materials, and imaging needed to obtain insurance coverage. Review various appliances and alternatives for sleep disordered breathing or OSA

What we will need

  1. Sleep Study- With PSG and evaluation (completed within the last 10 years). Insurance companies vary on this.
  2. Prescription from Physician for a Mandibular advancement device, oral sleep appliance. Letter of medical necessity
  3. Current Insurance Card
  4. Driver’s License/ ID
  5. You will be given additional paperwork to fill out in office.

Insurance Verification And Processing

If you qualify and decide to proceed with the device: We will run a soft pre authorization with insurance to verify benefits.  Once the soft preauthorization is run, you will be given a quote and estimated portion. We will then run an official preauthorization with insurance, once complete we can proceed. ½ patient portion is due at records appointment and ½ due at delivery.

1 Hour Appointment For Records And Comprehensive Exam:

charting of gums, CBCT, photos, digital impressions and records for bite, baseline full mouth x-rays and a comprehensive TMJ and muscular exam.

-If teeth and gums are healthy and do not require work, we will proceed with oral sleep appliance

-If dentition, periodontal health is not sound, needs work a provisional/ temporary sleep appliance will be made and you will be required to complete dental treatment Prior to fabricating a device. This will be done with their current dentist, if they don’t have a dentist, work can be done at our office.

45 Min Appliance Delivery Appointment

We will deliver device, review how it is used, verify proper fit and comfort.

Follow Up Appointments

-1 day to 1 week follow up: Slowly advance mandible based on your needs, tmj, comfort.

-2 week follow up–1 month follow up

– continued care and maintenance.

Return to your sleep physician or PCP to evaluate the effectiveness of your appliance.
